Families and Visitors

Vision:
To provide a local opportunity for staff, administrators and patients and family members to come together in interdisciplinary teams to learn, develop and share skills in Patient & Family Centered Care.

Purpose:
To bring together professionals, patients and family members from hospitals already engaged in PFCC activities with those just starting out with PFCC programs, and those wanting to initiate a PFCC program, to provide a peer learning opportunity as well as an opportunity to learn from experts in the field.

Activities:
Conference participants will have the opportunity to learn from experts in the field of PFCC. All levels of PFCC program development will be presented from selling the concept to forming Family Advisory Councils to plans for sustainability. Additional time will be provided during a Topic Table lunch to network with others doing this work in Southern California.

Participants:
We encourage institutions to send teams consisting of staff, family advisors and Administrators. The Interdisciplinary approach is a central component of Patient & Family Centered Care, which includes a patient or family representative. Nurses, Therapists, Spiritual Services, Physicians, Social Workers and Palliative Care Representatives are all encouraged to attend. Hospital Administrators are also encouraged to attend, as their role in supporting PFCC efforts is recognized as critical to the success of every PFCC program.

Learning Objectives

  • Participants will develop or deepen an understanding of the fundamentals of Patient & Family Centered Care.
  • Participants will be able to objectively evaluate their own institution regarding PFCC practices.
  • Participants will have the tools and map for starting successful Family Advisory Councils.
  • The foundation for an on-going collaborative for the support of PFCC practices in Southern California will be established.
  • Patients & Family members will receive training specific to their role in PFCC and have resources to further their training.
