Neonatal Transport Program
The Miller Children’s Hospital Long Beach neonatal transport program makes transferring a neonatal intensive care patient easy for referring hospitals and physicians. The Miller Children’s multi-disciplinary neonatal intensive care team coordinates all aspects of the transfer—they secure the ambulance, arrive with the neonatal transport team and transport the critically ill baby to Miller Children’s level III neonatal intensive care unit (NICU), the largest in Los Angeles, Orange and San Diego counties.
Checklist for the Referring Hospital
The referring hospital can help expedite the transfer-of-care process, by having the following documents ready for the Miller Children’s neonatal transport team.
Two copies of the face sheet Copy of maternal record, including prenatal labs and H&P Copy of delivery record Copy of infant’s chart: Admission record Transfer/discharge summary Medication record, including orders and administration history Lab results Ultrasound/X-ray films/CD Flowsheet California Perinatal Transport form (CPeTs)
